Download and complete the Application for Membership. Attach copies of credentials: Michigan License, Diplomas, Board Certifications and Sponsors. Sign application, include payment based on the category of membership and mail to:

All applications are subject to review by the MSP Membership Committee. Application processing times may vary. All applicants are notified via email, regarding the status of the application within 45 days of submission.
